For a specific language that you can use in the debriefing form, please read our debriefing form template (general or deception). Few surveys are really considered anonymous. Even if a participant`s name is not requested in the survey, other information (IP address, email address, postal code, etc.) and/or demographic issues (gender and race, especially in a small, less diverse sample) can be used to determine the identity of individual participants. Even in a personal survey, even if a name is not registered, the mere fact that the researcher meets the participant face-to-face denies the concept of anonymity. A more accurate description of the survey data would be that it is confidential, meaning that researchers use certain procedures to maintain the confidentiality of participant data. Some research requires a debriefing after participants have completed an online survey. Online debriefing forms should be similar to the debriefing process performed in laboratory experiments. The debriefing page should appear immediately after the last question about the survey. Participants should be thanked for their participation and further information on the purpose of the study should be provided. In addition, contact information and information on other resources (IRB information, health services, local resources) should be provided to researchers, and participants should be reminded to print a copy of the debriefing form for their records. Participants should also be able to withdraw their data at that time (now that they have been fully informed of the intent and purpose of the study).
If they agree to have their data used for the study, they should have an “I agree” button to click on it and submit their data online. If they do not agree to their data being used in the study, they should have a “I disagree” button to click so that their data is not transmitted and collected online. Please check with the online survey program you are using to see if these features are allowed. For specific wording that can be used in the consent form regarding privacy and data security, please read our consent form templates. Some surveys may not require signed consent. For surveys where the risk to participants is minimal, where the consent signature is the only identifying information collected, and/or for online surveys, it would be preferable to use a simple consent paragraph rather than the much longer signed consent form. Please note that even if participants are not required to sign the consent form, consent will still be obtained and participants should still receive the same type of information (voluntary nature of the study, risks, benefits, confidentiality procedures, etc.) as participants who physically sign a consent form. The abbreviated paragraph on paper consent or consent to the online survey serves as a consent document for participants, and the process by which participants access and complete the survey constitutes consent. Click here for instructions on informed consent from the Office for the Protection of Human Research (OHRP). If you prefer to write your own consent document, you can do so, but be sure to include all the necessary elements of informed consent. For online survey recruitment, please specify exactly how participants will be informed of the survey.
When emails are sent, please follow the appropriate IT and UMass policies and policies regarding bulk emails or emails to Listservs. Bulk email approval should be documented using the appropriate channels before a log submission is approved. You can find more information about recruitment on our website. Information Statement – Only for use in exempt online studies that are not anonymous. This approval bill has also been approved for use by the Medical College of Wisconsin, the Milwaukee School of Engineering, Marquette University, and The Children`s Hospital of Wisconsin. Combined Parental Consent and Consent Form for Children – Same as the standard consent form for adults; Use with children between the ages of 12 and 17 Request for waiver, waiver of consent documentation or change of consent – Complete page 1 if you do not receive consent or if you remove consent elements from the consent form. – Fill in page 2 if you do not collect a written signature on the declaration of consent For online surveys, special attention should be paid to how the data of participants is secured. This includes knowledge of the survey software used, the types of information collected (IP address, email address), the options offered by the survey software in relation to the information to be collected, how the information is stored and how the credentials are separated from the survey data, etc. It is important to note that external survey software companies (e.B Survey Monkey, Zoomerang, etc.) are different from the software licenses provided by UMass (e.B. Qualtrics, etc.), so the researcher should be aware of these differences and the impact this will have on the path and place of storage and maintenance of survey data. In addition to how survey software collects and manages survey data, the researcher must also provide information on how the data is managed once it has been retrieved by the survey software provider (e.B.
